TLC Service Request Online Form

The TLC online service request form is your access-point to the TLC's creative services. Please follow the instructions below to create a new service request.

NOTE: Please contact ITS Audio/Visual for ITS audio/visual requests (projectors and room/equipment set-up or loan).

We need to receive an authorized service request with an account code and signature in order to begin working on your project.

Please read these instructions before submitting an online service request for the first time. You can download/view a PDF of the instructions 

Step-By-Step Instructions to use the TLC Online Service Request Form

  1. Log-in to the Online Service Request using your SFU ID. If you are already logged into the SFU Authentication site, our system will recognize that you are already logged in.

  2. To create a new request, click “Create New”. You can also see a history of previously submitted requests on this page.

  3. Complete the form with all of the required information.  If Videography is selected, you will then be prompted to provide additional information about the event (e.g., date, times, deliverables). Please note that you can select more than one type of service. Relevant documents such as room set-up, previous service requests, logos or event schedules, can also be attached by selecting “Add Attachment”. You will be prompted to add a document after you “Save” the request.

  4. Click on “Review and Save”.

  5. At this point, you can review the submitted request for any errors or omissions. If you would like to change the submitted information, click on “Make Changes”, change the information, then click on “Review and Save”.

  6. Once the information has been checked, click “Save”.

  7. If you would like to attach files, you will be prompted to attach the files now. If not, you will be brought back to the service request-initiating screen, where previous and current service requests can be viewed.

  8. Once saved, you will be sent an email with the service request attached, as well as instructions regarding the submission of the work order. You will still have to print-out the completed form, provide an account code and authorizing signature, and return it to the TLC before our professionals begin any work.**

    **Ideally, we would have an electronic and automatic approval system in place for each submitted service request. We hope to incorporate this at a later time. However, due to financial approval procedures and financial reconciliation at the department level, we still request a signed service request form.

  9. Once you are finished, remember to logout of the system.
